    If you cannot be issued a death certificate after reporting a death due to missing data or evidence, you can have the notification of death certified.

    This certificate serves as proof that you have complied with your duty of notification.

    You can use the certificate to prove that the death has been duly reported. The notification is a prerequisite for the burial of the deceased person.

  • Requirements
    • The death must be reported by you to the registry office in whose district the death occurred.
    • The notarization of the death reported by you must be postponed by the competent registry office due to missing data or evidence.

    You can apply for the certificate of notification and deferral of notarization verbally or in writing at the registry office.
